This form is to register your child(ren) for Children's Religious Education at First Unitarian Church of Cincinnati, including Sunday Infant & Toddler Care, PK-6 Sunday School, and 7-12 programs. One registration form is required per household. 

For the safety and well-being of all children, it is imperative that staff has the following information on file. Please complete all relevant boxes to the best of your ability, including responding "None" where prompted.

Completing all boxes may feel tedious, but it helps us sort registration data efficiently and effectively. Thank you for your time and support.

We recommend you complete this form on a desktop or laptop. Users find the formatting to be difficult to follow on phones.  

This form accommodates up to 3 heads of household and up to 4 children. If your needs exceed these amounts, please contact dre@firstuu.com

Complete head of household information for those who will be emergency contacts for children while at First Church. This applies to the adults most likely to accompany their children while on church grounds. 

In addition, complete head of household information for those who should receive information regarding Children's Religious Education news and updates. All listed heads of household will be included on our email list.

For example,
- Two parent households, please include information for both parents as emergency contacts. If one parent does not attend church, do not mark them as an emergency contact.
- Grandparents who will be taking grandchildren to church, please register yourself as the emergency contact. If your spouse might accompany children on their own, list them as an emergency contact as well. If desired, include a parent's information as an additional household member who should receive news and updates via email.
- If you are a single parent, you need only complete one head of houseld information as emergency contact.  If there are adults you would like to include on our email list, submit their information as well.
